Speaker to discuss advanced care planning

Heartland Hospice and Crystal River Nursing and Rehabilitation Center will host two presentations on Tuesday, April 16, in support of National Healthcare Decisions Day.

The presentations will be held from 9:30-11 a.m. and from 6:30-8 p.m., at Trinity Lutheran Church, 206 E. Badger St.

Free information and tools will be available to assist individuals in talking with family, friends and healthcare providers about their wishes.

Healthcare professionals will also be available to answer questions about advanced care planning and the necessary documentation.

Heartland Hospice and Crystal River Nursing and Rehabilitation Center will be joininig a nationwide effort to highlight making decisions about advance healthcare.

Adult age groups should consider having two forms completed for documenting health decisions, including a health care power of attorney

and a living will.

Both of these documents will be available to complete at the presentation and will be free of charge.
